Civil Cover Sheet (Form JS 44) has been revised. The new version will be required as of July 1, 2016 when filing an Initiating Complaint.

The updated JS 44 form is available on the Forms section of our web site.

Based on a recommendation from the District Clerks Advisory Group, the AO’s Judiciary Data and Analysis Office has developed a method to enhance the tracking and reporting of multidistrict litigation (MDL) cases. This method will allow the AO and the courts to better identify MDL cases that originate or are directly filed in the same court managing the master MDL Docket (i.e., the transferee court), in contrast to MDL cases that are transferred to a court from another district court. The method includes the use of a new origin code, updates to the Civil Cover Sheet (Form JS 44), and configuration changes to the District Case Management Electronic Case Files (CM/ECF) application.
