The Attorney Status Inquiry Form requires that an attorney's Pennsylvania Bar ID Number be included. That number can be obtained from the Pennsylvania Disciplinary Board website, https://www.padisciplinaryboard.org/for-the-public/find-attorney (link is external). If no Pennsylvania Bar ID Number is available, zeros or an attorney registration number issued by a different state can be entered in this field (e.g., NJ12345 or CA56789).
